Are your hip and knee outcomes ready for TEAM?
About ten taps on how your office collects outcomes today. You get a gap list against the measure CMS uses for hip and knee replacement in Hospital IQR and TEAM, plus where the ACCESS Model touches your non-surgical patients. Each line links the CMS document it comes from.
The questions are about your process, never about a patient. Your answers stay on this page and are not sent anywhere. It guides; the hospital submits, and the surgeon decides and signs.

Capture a KOOS JR or HOOS JR on synthetic answers, scored with the official table. The surgeon signs and a hash seals exactly what was captured, so anyone can check later that nothing changed.
What CMS requires, with sources
Sources last read 2026-11-02. If a rule changes, the CMS document wins over this page.
- The hip and knee replacement PRO-PM (CMIT 1618) collects the HOOS JR for hips and the KOOS JR for knees, for Medicare fee-for-service patients 65 and older having an elective primary inpatient hip or knee replacement. CMS Hospital IQR webinar on PRO-PM voluntary reporting, Sep 2022
- The preoperative survey is collected 90 to 0 days before surgery. The postoperative survey is collected 300 to 425 days after surgery. CMS MMS Hub: THA/TKA PRO-PM public comment summary, Feb 2022
- Hospitals must have complete preoperative data with matching complete postoperative data for at least 50 percent of their eligible hip and knee patients (ASCs: at least 45 percent). CMS Hospital IQR support contractor Q&A on the THA/TKA PRO-PM, Sep 2024
- Before surgery the measure also collects risk variables: the mental health items of PROMIS-Global or the VR-12, the SILS-2 health-literacy question, BMI or height and weight, chronic narcotic use, back pain, and pain in the other joint. CMS Hospital IQR webinar on the THA/TKA PRO-PM, Sep 2024 (transcript)
- The hospital reports the measure through the HQR system (as CSV, XML or a web form). CMS's support contractor encourages hospitals and ASCs to work with physician offices to share the data. CMS Hospital IQR support contractor Q&A on the THA/TKA PRO-PM, Sep 2024
- Hospital IQR reporting is mandatory starting with procedures from July 1, 2024 to June 30, 2025, for the FY 2028 payment determination, after two voluntary periods. CMS Hospital IQR webinar on the THA/TKA PRO-PM, Sep 2024 (transcript)
- TEAM runs from January 1, 2026 to December 31, 2030. It scores inpatient hip and knee replacement episodes on the same PRO-PM, using the hospital's IQR data; hospitals report nothing separate for TEAM, and a hospital that does not report will not receive potential reconciliation adjustments for high-quality performance. CMS TEAM quality measures fact sheet
- TEAM performance year 1 uses procedures from July 1, 2024 to June 30, 2025; year 2, July 1, 2025 to June 30, 2026; year 3, July 1, 2026 to June 30, 2027. CMS TEAM quality measures fact sheet
- The ACCESS Model runs from July 5, 2026 to June 30, 2036. Its musculoskeletal track covers chronic musculoskeletal pain lasting more than three months in Original Medicare. CMS Innovation Center: ACCESS Model
- ACCESS's musculoskeletal measure collects PROMIS Physical Function and PROMIS Pain Interference for every patient, plus one site-specific survey (KOOS JR for the knee, HOOS JR for the hip, or QuickDASH, ODI or NDI), plus a 0 to 10 pain rating. CMS ACCESS Model request for applications
- ACCESS's improvement target for the KOOS JR and HOOS JR is a 10 point increase; payment is reduced when fewer than 50 percent of a participant's patients meet their targets. CMS ACCESS payment amounts and performance targets
- ACCESS collects the baseline within 60 days of alignment and the end-of-period survey no later than 425 days after alignment, each submitted within 15 days of collection through a FHIR-based API. CMS ACCESS payment amounts and performance targets
- Any Medicare Part B-enrolled practitioner can refer a patient to an ACCESS participant, and the participant sends the referring clinician updates at the start of care, on escalation and at completion. CMS ACCESS: for referring clinicians
